Portable space heaters can create fire hazards and place excess load on electrical systems. For safety and code compliance, all space heaters used on campus must be reviewed and approved before use.

Before submitting this form, you must complete two separate work orders with UNT Facilities (940-565-2700) and obtain both work order numbers.

1. Request a Cold Call / Temperature Check through Facilities

Facilities will visit your workspace to evaluate the temperature and adjust the HVAC system if possible.

If the space is still too cold after 1–2 days…

2. Request an Electrical Site Survey

After the Cold Call/Temp Check is completed, submit a second work order for an Electrical Site Survey.

Facilities will determine if the circuit can safely support a 250-watt maximum space heater.

Once the Electrical Shop approves the use of a space heater, submit this form. Both work order numbers must be included. A fire/life safety representative will contact you to schedule a time to place an approval sticker on the unit.
