Instructions for submitting a permit request:
- If possible, consider seeking an alternative or safer method for your work.
 - Form must be submitted at least 24 hours before the work is to begin
 - Read all the Mandatory Precautions for hot work under Hot Work Permits section of the RMS website
 - You must also contact UNT Fire Systems at 940-565-2700, at least 24 hours in advance, for fire detection/suppression considerations.
 - All information in the form is required.
 - All permits are only valid for the specified dates and times requested.
 - A mandatory inspection will be conducted by our office to verify compliance with safety precautions.
 
After your submission:

 - Upon approval of your request, you will receive an email notification with a link to your completed form and a printable PDF of your approved permit.
 - The approved permit must be printed and displayed at the work site at all times.
 - After hot work has been completed, send signed Daily Hot Work log to FLS@unt.edu.

Upon submitting the request, the requestor or contractor must contact the Fire & Life Safety Office (FLS@unt.edu) to schedule a time for on-site inspection of the hot work equipment setup and fire extinguisher.
