Hot Work Permit Permit Request

Instructions for submitting a permit request:

  1. If possible, consider seeking an alternative or safer method for your work.
  2. Form must be submitted at least 24 hours before the work is to begin  
  3. Read all the Mandatory Precautions for hot work under Hot Work Permits section of the RMS website
  4. You must also contact UNT Fire Systems at 940-565-2700, at least 24 hours in advance, for fire detection/suppression considerations.
  5. All information in the form is required.
  6. All permits are only valid for the specified dates and times requested.
  7. A mandatory inspection will be conducted by our office to verify compliance with safety precautions.

After your submission:

  1. A mandatory inspection will be conducted by our office to verify compliance with safety precautions.
  2. Upon approval of your request, you will receive an email notification with a link to your completed form and a printable PDF of your approved permit.
  3. The approved permit must be printed and displayed at the work site at all times.
  4. After hot work has been completed, send signed Daily Hot Work log to FLS@unt.edu.

Upon submitting the request, the requestor or contractor must contact the Fire & Life Safety Office (FLS@unt.edu) to schedule a time for on-site inspection of the hot work equipment setup and fire extinguisher.
